The causes identified in the preliminary and detailed reports describes a minimum number of specialist(s) that should have been informed via email of the Event by providing to them the R-99. We are reiterating our requests, as follows: Emails that forwarded the R-99 to specialists, Proof of receipt of emails to specialist, title of Experts, and Division to which they were attached to.
