The latest (most recent-2022-07-19) IMM 5476 form dates from November 2021. In the context of adopting this new IMM 5476 form, IRCC has updated it's interpretation and officer instructions of what constitutes a correctly completed IMM 5476 form. Notably, we've seen files where IRCC refuses to add and/or remove a representative because the same form is used for both operations, and we've seen IRCC refuse to use the same IMM 5476 form for several applications by the same applicant, requiring a different IMM 5476 form for each application, restictions which IRCC was not applying in the past. These changes seem to have occured around the adoption of the latest IMM 5476 form in november 2021. However, we can suppose the relevant period for records is from January 1st 2021 to July 15th 2022. I would please request a copy of these new instructions or this new interpretation of what constitutes a complete and well-filled IMM 5476 form as they were provided to officers and circulated within the department (which were probably circulated along with the IMM 5476 form when it was released in November 2021, and/or were posted to an intranet) as well as copies of all efforts made by IRCC to inform its partners and stakeholders in private communcations, in particular professionnal representatives and their professional bodies, of these changes.
