I would also like to receive copies of all documents that are (or will be or have been) used to review and analyze accommodation requests, whether medical or religious (template, evaluation table, criteria, etc.). I would also like to receive copies of the documents that will be used to determine or assess the "sincerity" of religious beliefs or vaccine exemptions that non-vaccinated employees must file.
