All VAC documents related to ALL of the protocols, policies , procedures related to the "VAC rehabilitation program" in which VAC Case Managers and VSTM's are expected to follow. In particular, when a vet member is being placed on the rehab program and but most IMPORTANTLY removed from this program. What is the actual criteria for removing said Client. What are the collaboration rules and policies and specific 100% requirements that VAC case managers are directed to follow when initiating, planning, communicating and planning rehabilitation disengagement with the veteran. Is there a checklist of what protocols, policies & procedures are to be followed, yes or no ? And should there be. Also requesting to know everything a VAC case manager and VSTM is expected to do in the course of their duties "collaborating rehab disengagement" with said veteran. Is it VAC policy to engage and include the Veteran with this process? yes or no and if yes how is this accomplished as per VAC policy? Lastly, is a VAC case manager expected to review the vet members rehab goals with their vet client in the course of performing their duties at regular intervals and prior to disengagement, yes or no? If yes or no please provide the expected criteria that VAC staff follows discussing the Vet members rehab goals.
