During a presentation on Nov. 3, 2022, CNSC staff delivered a risk assessment relating to deterioration of pressure tubes. CNSC staff concluded that Level 3 Defence in Depth "is not adversely afffected by the type of pressure tube failure that could potentially result from reduced fracture toughness." Also, they determined that "the increase in risk metrics such as severe core damage frequency (CDF) and large release frequency (LRF) due to postulated increases in pressure tube failure frequency is negligible for 2-3 years of continued operations." I request all reports or other documents possessed by the CNSC detailing how these conclusions were arrived at. Provide every final Harassment and Violence investigation report completed between April 1, 2021 to March 31, 2023. These reports are prepared in accordance with the Work Place Harassment and Violence Prevention Regulations. They are typically held by Human Resources officials (e.g., OHS, Labour Relations, Values and Ethics) in headquarters and regions. Section 30 (2) of the Work Place Harassment and Violence Prevention Regulations states that: "An investigator’s report must not reveal, directly or indirectly, the identity of persons who are involved in an occurrence or the resolution process for an occurrence under these Regulations."
